No student-athlete participating in NCAA Division II sports while at UH Hilo, will be allowed to practice, compete or participate in any UH Hilo sponsored athletic activities, until all forms have been completed either online or received. Sports Information/Bio Sheet (Deadline to complete the online form is July 20) (Sports Medicine forms will also be required by our Sports Medicine Office.) Whether you are a “new” or “returning” student-athlete, you are required to review and/or complete the forms in all sections below, by the required deadline. Your questions and concerns are of the utmost importance, and we encourage an “Open Door Policy” so please feel free to email us or stop by our offices. If you have questions about the NCAA or Pacific West Conference rules and regulations, our UH Hilo Athletic policies, or initial and continuing eligibility, please contact your Head Coach, our Compliance Officer or any Administration Athletic Staff member for directions and assistance.
